Full Time - Patient Navigator – Poughkeepsie, Washington St
Sun River Health is a Federally Qualified, Non‑Profit Health Center serving underserved and vulnerable communities across the Hudson Valley. We provide comprehensive primary, preventative, and behavioral health services to all who seek care, regardless of insurance status or ability to pay.
We are seeking a Full‑time Patient Navigator to support our Poughkeepsie Washington St site. The Patient Navigator identifies community members in need of preventive and primary health care services, links them to the services they require, and mentors the Patient Representatives.
Essential Duties and Responsibilities- Consult with medical staff to link patients to services, providing ancillary support such as translation, transportation assistance, and application assistance.
- Facilitate patient appointments across SRH departments and external agencies; reference SRH preferred provider lists.
- Assist patients in overcoming obstacles preventing needed referrals.
- Track referrals and progress in eCW, alerting providers when appropriate.
- Answer patient questions to clarify provider instructions, procedures, and referral needs.
- Transmit necessary clinical information to specialists for referral; obtain consent for release of information.
- Review open referrals for upcoming medical appointments and provide updates to the clinical team.
- High school diploma or GED (minimum education). Bilingual in Spanish preferred.
- 0–6 years of relevant experience in patient navigation, case management, or primary health care.
