Genesis Patient Navigator

Job Description

Summary:

The Genesis Patient Navigator will work as part of a specialized team at Sun River Health. Responsibilities include assessing patients' needs and barriers to care, coordinating care with an interdisciplinary team, and providing education and support. The Navigator uses motivational interviewing and a harm reduction approach to deliver trauma‑informed care.

Full‑time. Hourly pay $22.00–$24.00. Sign‑on bonus $1,500. Terms and conditions apply.

• Assist patients with referrals for specialty care and community resources to address social determinants of health.
• Use sound and ethical judgment to maintain effective relationships with individuals and organizations.
• Assist in coordinating patient psycho‑education groups.
• Facilitate patient appointments within SRH and outside agencies following SRH procedures and provider lists.
• Consult with medical staff to link patients to ancillary services such as translation, transportation, and application assistance.
• Help patients overcome obstacles preventing them from receiving referrals.
• Answer patient questions to clarify provider instructions, procedures, and referral needs.
• Review open referrals for upcoming medical appointments and provide to clinical team.
• Conduct referral follow‑up and document outcomes in the EMR in compliance with program policies and standards.
• Educate patients on importance of medication and medical adherence, U=U, harm reduction, and safer sex practices.
• Participate in trainings to increase knowledge of HIV/AIDS and related topics, including completion of Peer Certification Program.
• Demonstrate cultural competency with diverse populations using an anti‑racist framework and equity lens.
• Conduct outreach to re‑engage HIV+ patients in care.
• Maintain progress notes and case records as required in EMR and relevant grant‑funded data programs.
• Accompany patients to appointments and/or lab services as needed.
• Participate in case conferences, QI activities, and other agency or community‑based meetings or trainings as assigned.
• Collaborate with multidisciplinary care team to improve patient health outcomes.
• Participate in quality improvement trainings and activities to improve patient care.

• High School Diploma or GED required.
• 2–4 years of related work experience.
