Title:
REGISTERED NURSE
Schedule:
3 shifts per week, 12.5 hours, overnight (weekends and holidays included), 7:00 PM–7:30 AM.
Payrate: $44.50/hr

Location:

Provide nursing services to Hospice patients and their families and to ensure that those services are consistent with the philosophy, goals, objectives and policies of the Agency.
Essential Functions- Perform initial and ongoing patient/family, physical, emotional, psycho-social, cultural and spiritual assessments to ensure and coordinate the delivery of comprehensive, holistic services by the Interdisciplinary Team.
- Provide palliative nursing care and symptom control in accordance with physician orders and IDG plan of care.
- Documents and enters into EMR all assessments and interventions before the end of the shift.
- Develop and prioritize the interdisciplinary care plan goals of the patient/family, hospice team and attending physician, prior to team meetings.
- Update the care plan a minimum of every 7 days.
- Provide anticipatory guidance to the patient/family to assist in preparing for coping with events as they occur.
- Teach, supervise and counsel patients/families/caregivers regarding physical care, observing return demonstration of procedures as appropriate for safe discharge planning.
- Effectively communicate patient status to family members and or caregivers.
- Revises nursing diagnosis, goals and interventions, expected outcomes, and plan of care based on assessment of data and input of patients and family members prior to the weekly IDG meeting.
- Provide Coordinator or Executive Director staff report on all changes in patient/family condition and POC.
- Follows and upholds all state and federal laws and regulations outlined in the policy and procedure manuals.
- Provide comprehensive end of shift report to oncoming staff.
- Ability to work non-judgmentally with patient/family of any culture, religion, socio-economic background or life-style.
- Exercise critical thinking skills
- Ability to effectively communicate verbally (in person and by phone) and in writing.
- Capability to work effectively and cooperatively independently and as a team member.
- Skill to prioritize and handle multiple tasks efficiently.
- Utilized good triage skills. Able to prioritize patient needs.
- Effectively communicates assessment with providers and other IDG members.
- Work in fast paced environment
- Ability to lift at least 30 pounds of supplies and/or equipment
- Moderate/High level of possible exposure to infectious agents.
- Routine duties involve potential for direct exposure to blood or body fluids.
- Graduate of an accredited nursing school.
- One year of hospice experience preferred.
- Two years nursing experience preferred.
- Venipuncture and IV placement certification preferred
- Minimum one year medical-surgery experience preferred
- Valid New York State nursing license.
- Valid New York State drivers license.
- Valid BLS Certification
