RN Case Manager - Home Health

• Assessment of physical, emotional, socio‑economic needs and environmental needs of patients and their families.
• Provide services ordered by the physician as indicated in the plan of care and communicate with all physicians and other health care practitioners as appropriate.
• Develop, implement and evaluate the plan of care in partnership with the patient, representative (if any), caregiver and physician(s).
• Coordinate services with the patient, family and others involved in the care plan. Provide patient, caregiver, family counseling and education.
• Regularly evaluate patient progress, take prompt action when a change in the patient’s condition is noted or reported, and terminate care when goals of management are attained or when termination is required according to agency policy.
• Participate in the agency quality assurance and performance improvement program and agency‑sponsored in‑service training.
• Identify patient and family needs for other home health services and refer when appropriate.
• Supervise services rendered by the homemaker/home health aide in accordance with state, federal and Joint Commission standards, and submit periodic evaluations.
• Fulfill all compliance responsibilities, including preparing clinical notes and attending in‑service training.

Required:

RN diploma with over 3 years of job‑related experience will be considered in lieu of an Associate’s Degree. Current RN license in the State of New York. Strong research skills, clear and concise interpersonal communication skills, and knowledge of systems integration.

Minimum Experience:

one year.

Experience in a qualified home health agency.

Work Type:
Full‑time. Standard hours: 40.00.

Work schedule:

day 8. Shift: Mon‑Fri, 8:30 AM‑5:00 PM with weekend rotation.
