Position Summary

A Licensed Practical Nurse renders nursing care to patients, within the unit of the Center or authorized community site, in support of medical care as directed by the medical staff and pursuant to the objectives and policies of the Center. The candidate will perform job duties in compliance with systems and job‑specific standards, and will complete additional duties or tasks as assigned.

Pay Range: $29.00 - $33.02 per hour.
Sign‑On Bonus: $2,700 for eligible external applicants – terms and conditions apply.

Essential Duties and Responsibilities
  • Interview and maintain patient’s medical record to include nursing observation, health history, vital signs, vision/hearing tests, medications and treatments given, reactions to tests (e.g., PPD measurements), laboratory test results, and utilize patient identifiers. (PEAs, PhQ, Pain, Smoke Status, etc.) Use CDSS and ensure that CVRs are completed as needed.
  • Perform in‑house labs, collect specimens for outside labs, and perform quality control in accordance with organizational protocols.
  • Complete and update all assigned tasks and logs.
  • Initiate follow‑up as necessary (as directed by provider), including encounter, labs, and close non‑billables.
  • Provide health teaching as indicated by physician, diagnosis, or patient inquiry; direct patient to Registered Nurse as necessary.
  • Collect data on patients under the supervision of the Registered Nurse or provider. May not triage; may not interpret patient clinical data or act independently on such data.
Essential Job Qualifications
  • Minimum Education Requirement:
    Vocational/Technical School/Diploma Program – Nursing
  • Minimum Licensure:
    • Valid New York State LPN License
    • CPR Certification
    • Infection Control Certification
