Salary: $60,000 – $120,000 per year (based on experience and schedule)

Part‑Time or Full‑Time. Flexible hours. No nights or weekends required. On‑site at assisted living communities.

Qualifications
  • Master’s Degree in Nursing from an accredited NP program
  • Active unrestricted New York NP license
  • DEA registration for prescribing authority
  • Eligibility for Medicare/Medicaid billing
  • 3+ years of clinical experience, preferably in geriatrics or long‑term care
  • Reliable transportation to travel between assigned communities
  • Compassionate bedside manner, teamwork, and sense of humor
Responsibilities
  • Conduct comprehensive health assessments, physical exams, and medication reviews
  • Develop personalized care plans in collaboration with interdisciplinary team
  • Manage chronic and acute conditions within NP scope
  • Order and interpret lab results and diagnostic tests
  • Prescribe medications responsibly
  • Educate patients and families on preventive care, wellness, and independence
  • Maintain accurate, compliant documentation in eClinical

    Works (eCW)
  • Participate in quality improvement and chronic care management initiatives
