Compensation: $60,000 – $120,000 per year (based on experience and schedule) + $5,000 Sign‑On Bonus.

Schedule: Part‑time or full‑time, flexible hours with preferred shifts Monday, Wednesday, Friday 9 AM – 2 PM. No nights or weekends required. Position is on‑site at assisted living communities.

• Master's Degree in Nursing from an accredited Nurse Practitioner program.
• Active, unrestricted New York NP license.
• DEA registration for prescribing authority.
• Eligible for Medicare/Medicaid billing.
• Minimum 3 years of clinical experience, preferably in geriatrics or long‑term care.
• Reliable transportation to travel between assigned communities.
• Compassionate bedside manner, strong teamwork skills, and a good sense of humor.

• Conduct comprehensive health assessments, physical exams, and medication reviews.
• Develop personalized care plans in collaboration with the interdisciplinary team.
• Manage chronic and acute conditions within your NP scope.
• Order and interpret lab results and diagnostic tests.
• Prescribe medications responsibly.
• Educate patients and families on preventive care, wellness, and independence.
