Optum NY/NJ is seeking a LPN Discharge Coordinator to join our team in Middletown, NY.
LocationMiddletown, NY (Hybrid with visits to Garnet Health)
ScheduleMonday to Friday, 7:30 AM - 4:00 PM
Primary Responsibilities- Assess patient and family's unmet health and social needs
- Provide effective communications to improve health literacy
- Develop a care plan based on mutual goals with patient, family, and provider, including emergency plan, medical summary, and ongoing action plan
- Monitor patient’s adherence to plan of care and progress toward goals, facilitating changes as needed
- Facilitate patient access to appropriate medical and specialty providers and other care coordination team support specialists
- Ensure effective tracking of test results, medication management, and adherence to follow‑up appointments
- Facilitate communication between specialists and Primary Care Physician post‑discharge for cohesive care plan development
- Attend and actively participate in care coordinator‑related training and meeting activities
- Perform regular visits to provide patient and family support and education
- Active and unrestricted LPN license in the state of New York
- 10+ years of clinical nursing experience
- Experience in caring for chronic disease patients
- Experience with navigation of local medical and social support systems
- Experience in Care Coordination, Case Management, Home Health, Hospital, or Urgent Care
- Experience in clinical or community health settings
- Experience with Electronic Medical Records and Microsoft Excel
Hourly pay ranges from $20.00 to $36.00 per hour based on full‑time employment. Benefits include generous PTO, paid holidays, annual bonus potential, annual reviews, tuition reimbursement, and opportunities for career progression. Clinical licensure costs are covered.
