Location: City of Poughkeepsie
A community-focused housing organization in Poughkeepsie is seeking a Care Manager responsible for providing services to families and youth. You will develop service plans, facilitate referrals, and provide counseling, advocating for participants to help them achieve their goals. The ideal candidate has at least two years of relevant experience, strong communication skills, and a degree in human services. A valid NYS Driver’s License is required for this role, which may involve lifting and other physical demands.
