Housing Care Manager – May’s House

The Care Manager is responsible for providing services to families and individuals in their assigned program. These services may include providing general information about the program, crisis intervention services, developing comprehensive goal‑oriented service plans, providing guidance and support, facilitating referrals, and serving as an advocate on behalf of participants to assist them in securing services, entitlements and support to reach their goals.

Duties
  • Providing general information about the program, crisis intervention services, and screening applicants for acceptance into the program.
  • Orienting participants to the program and conducting intake interviews to collect information and assess the needs and strengths of program participants.
  • Developing comprehensive, goal‑oriented service plans in conjunction with the participant and, where appropriate, with family members, service providers and/or significant others.
  • Providing an appropriate level of guidance and support to help participants meet basic needs, address service‑plan goals, overcome challenges, and cope with disappointment.
  • Facilitating referrals and serving as an advocate on behalf of participants to help them secure needed services, entitlements and support.
  • Sharing information with program staff and collaborating with outside providers to ensure participants’ holistic needs are addressed in a coordinated manner.
  • Providing individual, group and family counseling in accordance with goals and participants’ case plans.
  • Facilitating or arranging workshops, support groups and other activities to help participants develop life skills, improve interpersonal relations, maintain healthy lifestyles, and enhance their overall well‑being.
  • Monitoring participants’ compliance with program rules and requirements, intervening to stabilize crisis situations and delivering positive or negative consequences in accordance with program procedures.
  • Monitoring progress toward service‑plan goals at regular intervals, establishing new goals and strategies as needed.
  • Participating in applicant interviews, case reviews, staff meetings, training sessions, supervisory meetings, committee meetings, agency events and other activities as directed.
  • Maintaining confidential participant files, case notes, records and reports as requested.
  • Assisting participants in preparing for discharge, developing a discharge and/or aftercare plan, and maintaining agreed‑upon follow‑up contact.
  • Providing general office support, operational support and assistance with overall program management when necessary.
  • Participating in planning efforts to evaluate program effectiveness, identify needs and trends, and develop strategies to overcome challenges and enhance program quality.
  • Obtaining ongoing training as required to effectively perform duties and support professional growth.
Qualifications
  • Education/Knowledge
    : BA/BS in human service field or equivalent combination of education, training and experience.
  • Experience
    :
    Two years of related work experience with a youth population; must have competent reading and writing skills along with strong communication skills.

Requirements

Valid NYS Driver’s License required.

Physical Demands

Must be able to climb multiple sets of stairs.
