Position Summary:
The Empower Case Planner is responsible for the delivery of casework services to families and children in care. The Case Planner is responsible for coordinating services with various agencies, professionals, and providers. The Case Planner represents the Agency at criminal and family court proceedings, school meetings, conferences with LDSS, and any other entity the Agency communicates with. The Case Planner maintains accurate and detailed case records and prepares reports and other documentation required by the agency and/or LDSS.

Monday-Friday 8am-5pm, Saturday-Sunday OFF.

- Paid time off includes 4 weeks of vacation, 3 personal days, 12 holidays and 9 sick days annually.
- BA/BS in related field
- At least 1 year of experience working with children and families
- Valid NYS Driver License in good condition as determined by the Agency
- Must be available to travel for home visits and court appearances
- Bachelor of Social Work (BSW) degree
- Bilingual in English and Spanish
- Previous experience with the service population
- Appropriate certifications/licensure
