Position: Program Case Coordinator (Grant Funded)

This is a 100% grant funded or externally funded position, for the duration as funding allows, and may be altered or eliminated contingent on program or alternative funding sources.

Under the supervision of the program manager, maintains strict standards of confidentiality, responsible for managing and coordinating assigned program caseload for the duration of program services for the assigned client; the assigned caseload may include both the individual client and/or family. Responsibilities include but are not limited to: performing intake assessments, perform danger assessment/safety planning as needed, assisting clients to connect to recommended services, supporting client in implementing clinical recommendations, and assist client with crime victim compensation applications and any other and assisting clients with problem solving and self-sufficiency skills to be able to implement clinical recommendations.

Assists with intake and scheduling of client interviews, exams, therapy, and any additional services required or requested. Consults with and provide support to program multidisciplinary team members; acts as a central point of contact for client and a liaison with community partners and additional MDT members while mutually supporting the client case;
Establishes contact with client and non-offending caregiver, and provides education about the coordinated Multi-Disciplinary Team (MDT) response;
Facilitates linkage and treatment referrals (both internal and external) to providers who specialize in providing treatment to clients who have experienced trauma;
Provides client and family with community resources and services not already in place (i.e. housing, protective orders, domestic violence intervention, food, crime victims compensation, transportation, public assistance, etc.). If applicable may accompany the client or family to specialized client services to act as a support person as needed (i.e. medical evidentiary exam or court related accompaniment). Manage emergency expense and transportation funding requests.

Maintains client records to reflect client interaction, service plan, and referrals made, Meets and networks with referral agencies to create and maintain a comprehensive provider network for clients and families;
Assist with coordination of monthly MDT Case Review, treatment planning meetings, and department meetings; provide updates and follow up with any recommendations of MDT;
Assists with coordinating/scheduling outreach and prevention trainings;
Attends regular staff meetings, case conferences and trainings as required;
Maintain and submit documentation required by funding and departmental requirements;
Perform other duties as requested by the Director and Manager, Maintains appropriate verbal, personal space, and relational boundaries with clients. Shall adhere to program competency training or continuing education requirements. Participates in the Camp Hope America and Pathways program and department continuation programing, groups and workshops. Participates as a team member of the clinical interdisciplinary team to ensure appropriate care.

Supports and provides resources to patients and families and displays on-going commitment to quality. Responsible for safeguarding the welfare of employees, patients, and visitors.

Ability to speak and reads English at a level that is sufficient to satisfactorily perform the essential functions of the position. Knowledge of standard office equipment (i.e., calculator, fax, photocopier) and personal computer and computer software skills (i.e., MS Windows, Excel, Access, Word, PowerPoint, internet, e-mail). Windows computer skills including proficient use of keyboarding, use of mouse or keys for functions such as selecting items, use of drop down menus, scroll bars, opening folders, copying and similar operations required upon employment or within the 1st two weeks of employment to perform the essential functions of the job.

Performs other duties as assigned. Follows Palomar Health rules, policies, procedures, applicable laws and standards. Carries out the mission, vision, and quality commitment of Palomar Health.

Job Requirements

Minimum Education: Bachelor's Degree in related field

Preferred Education: Master's Degree in social work or related field

Minimum Experience: 1 - 2 years related

Preferred Experience: 2 - 3 years related
Bilingual - Spanish preferred (Requirement Funding Dependent)

Required Certification: Verification of live scan filed with state licensing board (if applicable) must be submitted to HR OR new live scan must be completed within 14 days of hire prior to providing direct service
American Heart Association recognized BLS - Healthcare Provider

Preferred Certification: Danger Assessment;
Minimum 40 hour Advocacy Training (Requirement Funding Dependent)
Military Base Access (Requirement Funding Dependent)

Required License: Valid Driver's License

Preferred License: Not Applicable
