We are seeking a compassionate and detail‑oriented Clinical Reimbursement Coordinator MDS Nurse to join our team. The Clinical Reimbursement Coordinator (MDS Nurse) will play a critical role in assessing and documenting patient needs, coordinating care plans, and ensuring regulatory compliance. This position offers the opportunity to work in a supportive environment where your expertise will contribute to improving patient outcomes.

• Conduct comprehensive patient assessments to determine care needs.
• Collaborate with healthcare professionals to develop and update individualized care plans.
• Ensure compliance with Medicare and Medicaid regulations and guidelines.
• Accurately complete and submit MDS (Minimum Data Set) assessments.
• Monitor patient progress and adjust care plans as necessary.
• Provide education and support to clinical staff regarding assessment and documentation best practices.
• Participate in interdisciplinary team meetings to discuss patient care and coordination.
• Maintain accurate and up‑to‑date patient records.

• Current Registered Nurse (RN) license in Ohio.
• Minimum of one year of experience in long‑term care or similar healthcare settings.
• Strong knowledge of MDS assessments and Medicare/Medicaid regulations.
• Excellent communication and interpersonal skills.
• Detail‑oriented with a commitment to accuracy and compliance.
• Ability to work collaboratively in a team‑oriented environment.
• Proficient computer skills and familiarity with electronic health records (EHR) systems.
