Type:
Full-Time (40 hrs/week) M-F 8:00 - 5:00
Salary: $18.91-$19.84/hr DOE

- Coordinate Care:
Facilitate warm hand-offs and referrals between patients, providers, and community services. - Screen & Support:
Conduct SDOH screenings (PRAPARE) and help patients set goals to overcome barriers. - Educate & Advocate:
