One Health in Powell, WY is hiring a Community Resource Specialist to help patients navigate social services and connect them with vital supports. This 40-hour/week role runs Monday–Friday, with a pay range of $18.91–$19.84 per hour, DOE, and a benefits package including PTO.
You will be the frontline advocate, collaborating with care teams and community partners to improve access to resources and overall wellbeing for vulnerable members of the community.
