- DDS/DMD from a dental education program accredited by the Commission on Dental Accreditation.
- Current, valid state license to practice dentistry (license must be in good standing) or eligibility for licensure.
- New graduates are welcome to apply.
- Other certifications as required by state (e.g., CPR, DEA, etc.).
