Job Details

* 15 to 20 patients per day

* This is a part-time position

* Compensation is estimated range of $210,412 to $238,495 annually with no cap on productivity income potential; this full-time role is benefits eligible

Responsibilities

* Drive high-quality, safe, and cost-effective patient care in a post-acute care environment

* Skilled nursing facility and long-term care

* Collaborate with other clinician interdisciplinary teams and follow CMS guidelines

* Work closely with hospice, palliative care, and home care agencies for safe patient transitions

* Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines

Qualifications

* Board Certified FNP, AGACNP, ACNP, AGPCNP, or PA-C

* Willing to train experienced candidates as well

* Licensed in Arizona

* DEA required
