Key Responsibilities- Provide comprehensive hospice services in compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
- Conduct medical care, palliative management, and terminal illness care for patients.
- Complete history and physical examinations, evaluate medical condition, order and interpret lab and diagnostic tests, and document findings in the patient’s medical record.
- Develop individualized plans of care, prescribe medications and treatments, obtain consultations or referrals, and coordinate hospice services.
- Perform hospice face‑to‑face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility.
- Provide clinical information and recommendations to the hospice medical director regarding patient eligibility for certification and recertification.
- Coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners.
- Ensure symptom control and collaborate with interdisciplinary team members to improve patient comfort.
- Develop treatment protocols and propose interventions based on evidence and collaboration with the physician.
- Consult with the physician when a patient’s plan of care is outside standardized practice and protocols.
- Maintain knowledge of Medicare/Medicaid, state/federal regulations, and hospice service rules.
- Ensure compliance with Medicare conditions of participation and other state regulations governing healthcare provision.
- Comply with HIPAA requirements and organizational policies.
- Participate in quality monitoring, quality improvement initiatives, and organizational audit.
- Engage in personal professional growth through seminars, conferences, continuing education, and in‑service classes.
- Assist patients, families, and caregivers with continuity of care and provide nursing consultation and education to interdisciplinary team members.
- Build and nurture relationships with staff and community referral sources to facilitate program growth.
- Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
- Master’s degree with at least one year of Nurse Practitioner experience and at least one year of hospice or palliative care experience.
- Current nursing licensure in the state and CPR certification.
- Accreditation from an approved certifying body for advanced practice nursing as required by state.
- Preferred certification in a specialist area (e.g., Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics).
- Excellent observational, verbal, and written communication skills, problem‑solving abilities, and mathematical proficiency.
- Physical ability to walk or stand for prolonged periods, lift, transfer patients or supplies, and reach, stoop, bend, kneel, or crouch.
- Adequate visual and hearing acuity to perform required nursing skills.
- Licensed driver with an insured automobile in good working order.
