Home and Community Based Services Licensed Practical Nurse

As the Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care as prescribed by the physician to acutely and/or chronically ill patients in their homes, record clinical progress notes, and coordinate other care activities to deliver the highest quality care in your designated geographic area.

• Provide high‑quality clinical services within the scope of practice, following the plan of care and collaborating with other members of the health care team.
• Document patient visits thoroughly and in compliance with regulatory and payer requirements using the agency's electronic visit verification program.
• Actively participate in the agency's care coordination process, including reporting patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, other clinicians, agency management staff, case coordinators, and community case managers.
• Function in any home setting regardless of the client’s age, race, creed, color, sex, disability, or financial condition.

• Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

• CPR certification or willingness to obtain within 90 days of hire.
• Work flexible hours as required to meet client needs.
• Manage multiple tasks simultaneously and work independently.
• Good communication, writing, and organizational skills.

The hourly pay for this role will range from $22.90 to $34.35 per hour based on full‑time employment. Benefits include a comprehensive benefits package, incentive and recognition programs, equity‑stock purchase, and 401(k) contribution (subject to eligibility requirements).
