Seeking an experienced RN Case Manager to support acute care case management, focusing on patient care coordination, discharge planning, and regulatory compliance in a hospital setting.
Key Responsibilities- Conduct admission and ongoing case management assessments (medical necessity, level of care).
- Drive discharge planning and reduce length of stay through care coordination.
- Ensure CMS compliance, including admission reviews, continued stay, IMM & MOO.
- Coordinate transitions of care and post-acute placement.
- Maintain accurate and timely clinical documentation.
- Collaborate with physicians, nursing, and interdisciplinary team.
- Identify high-risk patients and prevent readmission.
- Support audits, compliance, and regulatory standards.
- Active RN license.
- Case Management experience in an acute hospital setting.
- Experience with Cerner EHR.
- Strong care coordination and discharge planning.
- Knowledge of CMS regulations and compliance.
