PRN-Occupational Therapist-OT | Preston, ID

Qualifications

• Occupational Therapist licensed in the state of Idaho.
• Current CPR certification.
• Valid driver’s license and auto insurance.
• Experience in home health and hospice settings preferred.

• Assume responsibility for the ongoing interdisciplinary assessment and development of individualized plans of care in partnership with patients, representatives, and caregivers.
• Provide services indicated by the physician per the plan of care.
• Offer patient, caregiver, and family counseling and education.
• Communicate with the prescribing physician and other health care practitioners regarding the home health plan.
• Evaluate patients’ functional status (muscle function, endurance, visual coordination, communication skills, self‑care ability, work capacity, etc.).
• Evaluate home environment for hazards or barriers to independent living.
• Identify equipment needs and, when appropriate, fabricate or instruct in the use of splints, adaptive or orthotic devices.
• Develop treatment programs and set goals for improved function.
• For patients returning to work, assess work capacity and refer for vocational training per policy.
• Maintain accurate clinical records, notes, and report changes in patient condition to the physician.
• Participate in the agency’s quality assessment, performance improvement, and continuing education activities.
