Job Description
Provides professional nursing services to patients and families by teaching and demonstrating nursing principles and techniques. Provides skilled care in the home, assisted living, or skilled nursing facility. This position is deemed safety-sensitive and will adhere to all policies associated with this designation. The duties listed below are intended only as illustrations of the various types of work performed. - Participates in ongoing interdisciplinary assessment
- Coordinates patient care and referrals
- Participates in developing and evaluating the point of care (POC) with the patient, patient representative, and the patient’s caregiver(s)
- Provides ordered services and assures that patient needs are continually assessed and met
- Provides patient, caregiver, and family counseling and education
- Completes accurate clinical notes using the electronic medical record
- Communicate with all physicians and healthcare practitioners involved in the POC
- Participates in the Quality Assurance and Performance Improvement (QAPI) program and in-service training
- Initiates appropriate, preventative nursing procedures and demonstrates and teaches health care to patients and families
- Participates in case management by integrating nursing services into the overall comprehensive care plan for the patient
- Plans for service, based upon knowledge of caseload, and determines priorities of patient service based on patient/family needs and resources, using professional judgment consistent with existing policies
- Observe, evaluate, and report to the physician the patient’s physical and emotional condition and reaction to drugs or treatment
- Interprets social and physical factors in the environment that affect patient care
- Maintains communication with the supervisor regarding daily visit schedules and relays patient information for appropriate disposition and follow-through to the physician or community agency
- Prepares clinical summaries, case conferences, and discharge summaries, as well as all recordings required in charting by the end of the month
- Records all nursing services, communication, and conferences, utilizing appropriate forms, promptly
- Provides written information to accounts receivable regarding changes in the Medicare status of patients
- Maintains necessary records and forms for analysis and planning of service to establish priorities for care
- Attend and participate in 10 staff meetings, eight in-service and continuing education programs
- Attends, participates, and may conduct professional conferences and at least one care conference per month
- Assigns staff based on specific patient needs, family capabilities, staff training, experience, and the degree of supervision needed
- Assists with patient assessment and Home Health aide delegation/supervision
- Provides written assignments/instructions for other staff involved
- May be required to organize a weekly schedule for all nursing staff
- Timely documentation, according to the current Franklin County Medical Center’s Timely Documentation Protocol, is required
- Maintain confidentiality by protecting sensitive, proprietary, or confidential information. Adhere to HIPAA privacy and security requirements
