• Licensed LPN in the State of Idaho.
• Current driver’s license and auto insurance policy.
• Reliable vehicle.
• Home health experience preferred.

• Provide direct patient care as defined in the State’s Nurse Practice Act.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
• Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse and/or Clinical Supervisor or Director of Nursing.
• Use equipment and supplies effectively and efficiently.
• Adhere to the organization’s documentation and care procedures and standards of personal and professional conduct.
• Participate in personal and professional growth and development.
• Perform other duties as assigned by the registered nurse.
