The purpose of this position is to treat and document care for residents with impaired ability to perform at an independent level due to disease process, under the supervision of a Physical Therapist, participate in the interdisciplinary team meetings as required.

DUTIES:
  • Follow physicians' orders for resident care and follow directions of the Physical Therapist

  • Provide appropriate treatment to help residents maintain or attain the highest level of function possible within the confines of his/her physical abilities

  • Provide appropriate treatment, preventative measures and adaptive equipment to prevent decline in residents' functions

  • Provide appropriate resident or caregiver education to help residents maintain or attain the highest level of function possible within the confines of his/her physical abilities

  • Maintain timely and accurate resident reports as required by federal, state and local regulatory agencies and third-party payers

  • Provide adaptive equipment and preventative measures to prevent decline in residents' functions

  • Observe infection control procedures

  • Be responsible for the safety of residents under his/her supervision

  • Consistently work cooperatively with residents, residents' representatives, facility staff, physicians, consultants and ancillary service providers

  • Follow all facility Residents' Rights policies

  • Remain positive and caring and act in the best interests of our residents

  • Perform all duties assigned in an effective, timely and professional manner

  • Perform other related duties as directed by the supervisor
