As the Team Manager
, you will guide and mentor staff, support clinical excellence, and drive efficient patient flow across the home health continuum. You’ll collaborate across disciplines to implement evidence‑based practices, lead operational improvements, and ensure high patient, staff, and partner satisfaction.

• Mentor and develop staff to support interprofessional, growth‑focused practice.
• Promote efficient patient flow and care coordination; identify and address operational gaps.
• Implement and disseminate evidence‑based clinical practices and quality improvements.
• Represent Penn Medicine at Home in the community; support outreach and student clinical experiences.
• Manage workflows, budgets, and financial performance to meet organizational targets.
• Ensure achievement of patient satisfaction and employee engagement goals.

• RN license (NJ) required.
• Bachelor’s degree in nursing or related field.
• 5–10 years progressive nursing or allied health experience (PT/OT/SLP).
• Home health experience required.
• Prior leadership experience required.
• BLS certification, valid driver’s license, and car insurance required.
