• Conduct comprehensive psychosocial assessments of patients and families to identify needs, barriers, and supports.
• Develop and implement individualized discharge plans that support safe transitions from hospital to home.
• Collaborate with hospital teams, caregivers, and interdisciplinary colleagues to coordinate services, resources, and referrals.
• Facilitate access to community supports, social services, and durable medical equipment as part of the care plan.
• Document assessments, plan of care, and progress notes accurately in the patient record; ensure regulatory and agency compliance.
• Provide patient and family education, grief support when appropriate, and ongoing psychosocial follow-up throughout the home care episode.

• Active Licensed Social Worker in New Jersey (LSW)
• Acute hospital experience required, specifically Med Surg and Emergency Department units
• Discharge planning experience is required
• Behavioral health experience will be rejected for this assignment
• Ability to perform in a home healthcare setting with communication and documentation responsibilities
