Case Manager RN Per Diem – New Jersey

The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care.
Case Manager RNs Are Responsible For The Following- Advocates for the patient and family throughout the entire episode of care.
- Monitors patient’s progression intervening as needed to ensure efficient patient progress.
- Coordinates all aspects of discharge planning and establishes a safe discharge plan.
- M-F 8:430
- 1 weekend day shift every 4 weeks
- 1 major holiday
- Willingness to occasionally float to all 3 campuses
- Required:
BSN - 2 years of medical surgical nursing experience
- Preferred: 1–2 years of acute care, homecare or population health RN case management experience
- Preferred: CCM certified
$37.65 to $53.67 hourly. The actual hiring rate will be determined based on candidate experience, skills and qualifications. This position is not eligible for an annual incentive.
