Schedule:
Full-Time | On-Call & Weekend Rotation

Compensation: $75,000 - $100,000 per year + $4,000 Sign-On Bonus

At Home Health Care, Inc. , we're looking for an experienced RN Visit Nurse who enjoys the independence of working in the field while still having an established clinical team behind them.

In a hospital or facility, your attention can be divided between multiple patients, competing priorities, interruptions, and everything happening around you.

Home health changes the environment.

When you arrive for a visit,
that patient is your focus.

You have the opportunity to assess more than what's written in the chart.

Is the patient actually taking their medications correctly?

Can they safely manage their wound care?

Does the family understand the treatment plan?

Has something changed since the last visit?

Is there a subtle clinical issue that needs to be addressed before it becomes an emergency?

That's why experienced nurses can thrive here.

As an RN Visit Nurse, you'll travel throughout your assigned area providing skilled nursing care directly in patients' homes.

Your visits may include:

• Wound care and wound assessment
• Medication reconciliation and management
• Injections and other skilled nursing procedures
• Monitoring chronic and acute medical conditions
• Patient and caregiver education
• Identifying changes in condition
• Communicating concerns and updates with physicians
• Coordinating with PT, OT, SLP, social work, and other members of the care team
• Helping patients understand how to manage their health safely at home
• Completing timely documentation in Homecare Homebase (HCHB)

• An active Minnesota Registered Nurse (RN) license
• Strong nursing assessment and critical-thinking skills
• Previous experience in home health, hospital nursing, rehabilitation, skilled nursing, or other clinical settings
• Confidence working independently in the field
• Strong patient and family communication skills
• Excellent organization and time-management abilities
• Reliable transportation and willingness to travel throughout an assigned service area
• A nurse who takes ownership of their patients and follows through

Previous home health experience is certainly valuable, but nurses from other clinical environments who have the right combination of independence, assessment skills, and patient-care experience are encouraged to connect with us.
