Compensation: $75,000 - $100,000 per year + $4,000 Sign-On Bonus

When you arrive for a visit,

Is the patient actually taking their medications correctly?

Can they safely manage their wound care?

Does the family understand the treatment plan?

Has something changed since the last visit?

Is there a subtle clinical issue that needs to be addressed before it becomes an emergency?

Your assessment can be the difference between a patient continuing to recover at home and ending up back in the hospital.

What Your Visits Could Include

As an RN Visit Nurse, you'll travel throughout your assigned area providing skilled nursing care directly in patients' homes.

Your visits may include:

  • Wound care and wound assessment
  • Medication reconciliation and management
  • Injections and other skilled nursing procedures
  • Monitoring chronic and acute medical conditions
  • Patient and caregiver education
  • Identifying changes in condition
  • Communicating concerns and updates with physicians
  • Coordinating with PT, OT, SLP, social work, and other members of the care team
  • Helping patients understand how to manage their health safely at home
  • Completing timely documentation in Homecare Homebase (HCHB)

  • An active Minnesota Registered Nurse (RN) license
  • Strong nursing assessment and critical-thinking skills

  • Excellent organization and time-management abilities
  • Reliable transportation and willingness to travel throughout an assigned service area
