MDS Nurse

The MDS Nurse is responsible for coordinating and completing the facility's Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) process in accordance with CMS, state, Medicare, Medicaid, and facility requirements. This position reviews clinical documentation, completes accurate and timely MDS assessments, coordinates Care Area Assessments (CAAs), and collaborates with the interdisciplinary team to develop and maintain individualized resident care plans.

The MDS Nurse works closely with nursing, therapy, dietary, social services, activities, physicians, and other healthcare professionals to ensure resident assessments accurately reflect each resident's clinical condition, functional status, diagnoses, treatments, and care needs. The position monitors assessment schedules and regulatory deadlines, identifies documentation discrepancies, supports accurate reimbursement, participates in care plan meetings and QAPI activities, and assists with regulatory surveys and audits.
