Position: Advanced Practice Professional- Hematology/Oncology (Princeton, WV)

Position Overview

The Advance Practice Professional (APP) is an individual with clinical skills and medical knowledge, specializing in the care of Hematology/Oncology (Hematology/Oncology) patients. The APP performs comprehensive patient health evaluations, including biological, social, and psychological assessments, and collaborates with the appropriate clinical/faculty team. The APP assesses educational and psychosocial needs, develops and reviews care plans, participates in patient education, and performs diagnostic and therapeutic procedures.

Duties and Responsibilities
  • Manage comprehensive and individualized care for patients in the infusion center and ambulatory setting.
  • Complete daily rounds; consult and/or pre‑treat or pre‑procedure evaluations in the infusion center as applicable and participate in ambulatory clinics.
  • Complete integrated assessments and order additional tests or consults according to established clinical guidelines and protocols.
  • Perform and communicate results of diagnostic tests, adhering to scope of practice.
  • Consult and communicate with clinic and hospital staff regarding abnormal findings.
  • Perform succinct comprehensive clinical evaluations and document findings in the medical record.
  • Facilitate patient discharge and/or follow‑up by ordering/coordinating appropriate services and tests.
  • Perform necessary procedures following established clinical guidelines and protocols.
  • Develop individualized care plans, including acute management, teaching, discharge planning, and referrals.
  • Demonstrate expertise in caring for patients across all ages.
  • Participate in physician rounds and inter‑disciplinary approaches to patient care.
  • Recognize emergency situations and initiate effective emergency care.
  • Evaluate follow‑up procedures and tests in the ambulatory setting.
  • Assist in the collection of outcome measures.
  • Utilize teaching/learning theory and strategies to educate nursing staff, hospital staff, students, patients, families, and the community.
  • Promote a learning environment that responds to patient/family preferences and readiness to learn.
  • Evaluate teaching effectiveness and collaborate with staff on education plans.
  • Maintain current knowledge base and complete all clinical competencies.
  • Serve as a resource person and provide educational programs to staff.
  • Mentor nursing and ancillary personnel.
  • Participate in community programs and distribute literature as appropriate.
  • Communicate effectively with patients/families, physicians, and ancillary services to ensure coordinated care.
  • Document plan of care, interventions, and evaluations consistently in the medical record.
  • Collaborate with leaders and staff to achieve patient outcome‑based improvements.
  • Provide leadership in implementing changes that positively impact patient outcomes.
  • Facilitate creative problem solving and professional support with medical and nursing staff.
  • Serve as a role model and promote innovation in care delivery.
  • Lead committees and task forces.
  • Document hours and submit billing reports as appropriate.
  • Participate in the UHA Performance Improvement Plan and apply improvement activities to enhance care.
  • Interpret hematology/oncology data and recommend/implement changes as necessary.
  • Participate in clinical trials and research studies.
  • Adhere to performance expectations in People, Service, Performance Improvement and Shared Values & Culture.
  • Assist faculty, administrators, and directors with outcome measure collection.
  • Oversee and provide individualized care in the infusion center, supervise patients until the last patient leaves.
Minimum Qualifications for Nurse Practitioner
  • Graduate of a CCNE accredited nursing program with a master’s degree in Nursing.
  • National certification by a recognized APRN Certification Center.
  • Licensure or eligibility for licensure as a Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
  • American Heart Association BLS certification required, with Advanced Life Support training at the discretion(s) of the department.
Experience
  • Hematology/Oncology, acute care, or internal medicine experience preferred.
  • Eligible new graduates will be considered.
