Superior Hospice in the Greater Twin Cities Metro Area is seeking a Hospice RN Case Manager to oversee an assigned patient caseload and deliver compassionate end-of-life care in home, assisted living, and skilled nursing settings.
You will collaborate with physicians, social workers, and the care team, provide pain and symptom management, educate families, and document care plans with accurate EMR entries. Must hold an active Minnesota RN license and thrive in a one-on-one patient care model.
