21-County Service Area (territories assigned based on coverage needs)

What You'll Do
  • Manage an assigned hospice caseload throughout the continuum of care.
  • Provide expert pain and symptom management focused on comfort and quality of life.
  • Serve as the primary Hospice RN Case Manager for patients and families.
  • Complete comprehensive nursing assessments and individualized care plans.
  • Educate and support caregivers through every stage of the hospice journey.
  • Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and community partners .
  • Provide care in patients' homes, assisted living communities, and skilled nursing facilities .
  • Maintain accurate EMR documentation and participate in a weekend and on-call rotation .
What You'll Bring
  • Active Minnesota Registered Nurse (RN) license.
  • Experience in hospice preferred , or a strong background in oncology, ICU, palliative care, home health, or other complex clinical settings.
  • Strong clinical judgment and the ability to work independently.
  • Exceptional communication and relationship-building skills.
  • A passion for patient-centered, compassionate hospice care.
  • Reliable transportation and willingness to travel throughout your assigned territory.
