Oasis Therapy Services is seeking a Speech-Language Pathologist to provide rehabilitation services in a senior care and skilled nursing setting in Prospect Heights, Illinois.
Employment type- Full-Time
EMR: Net Health
Responsibilities- Evaluate residents and develop individualized treatment plans with measurable goals.
- Assess and treat speech, language, cognitive-communication, voice, and swallowing disorders.
- Provide skilled therapy interventions to improve communication, swallowing, safety, and quality of life.
- Monitor residents' progress and adjust treatment plans as needed.
- Educate residents, families, and caregivers on communication strategies and swallowing precautions.
- Collaborate with nursing, physicians, rehabilitation professionals, and the interdisciplinary care team.
- Complete discharge planning and provide recommendations for continued care.
- Maintain accurate and timely Net Health documentation.
- Active and current Illinois Speech-Language Pathologist license.
- Master's or doctoral degree in Speech-Language Pathology from an accredited program.
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred.
- Current CPR certification.
- Strong communication, organization, and documentation skills.
- Geriatric, skilled nursing, dysphagia, or rehabilitation experience is preferred.
