The Providence Center Child & Family Clinical Therapist - Fee For Service will provide outpatient psychotherapy either at The Providence Center or in the community. Perform a comprehensive psychosocial assessment of assigned consumers.

• Complete crisis assessments, initial assessments, and integrated summary forms.
• Develop and implement a plan of care with ongoing consumer input.
• Provide individual mental health and/or substance abuse treatment and crisis assessment.
• Perform record keeping in accordance with Health Information Services and other Center requirements.

Master’s degree in social work or related field and independent license required (LICSW and/or LMHC).

Must have insured auto and valid driver’s license.
