SUMMARY
Under the general supervision of the Clinical Manager and in cooperation with the Director of the Sickle Cell Adult and Pediatric Program, this role assists in coordinating patient navigation activities for select at‑risk individuals with Sickle Cell Disease. The position provides both clinical and administrative oversight to program activities and patient care, and offers support to patients, families, and caregivers throughout their life span from pediatric to adult.

- Support patient comprehension of their diagnosis, treatment plan and next steps, and connect patients to the appropriate licensed clinical professional, as necessary.
- Support access to specialty care by assisting patients with scheduling appointments or locating available programs on their behalf.
- Coordinate patient transportation as needed.
- Support engagement in primary care including panel‑level reminder letters, phone calls and follow‑up.
- Identify and address non‑medical barriers to health and self‑sufficiency such as transportation, housing, income, education, medical coverage or other social determinants of health.
- Based on provider referral and patient screening, connect patients to available community resources such as reduced bus fare, taxi vouchers, housing support, income and food support, and job training.
- Establish and maintain positive relationships with community resources and social service agencies to link patients appropriately.
- Accompany patients in clinic and coordinate follow‑up care.
- Provide support within effective and realistic time frames.
- Assist with coordinating appropriate patient and family education and counseling regarding disease and treatment.
- Enhance patient satisfaction by responding to their support needs during illness.
- Participate in ongoing quality assurance activities, organizing meetings and follow‑ups.
- Collect and report information to ensure compliance with grant expectations and reporting requirements if appropriate.
- Participate in ongoing education and networking regarding navigation for patients with language barriers.
- Assist the nurse care manager in coordinating the care of patients identified as high risk and supporting individual care plans.
- Perform other duties as assigned.
Education: A Bachelor’s degree in a health‑related field preferred.
Experience: Experience working with vulnerable populations with at‑risk patients and their families in an informal or formal setting; ability to work compassionately with a diverse population; knowledge of relevant community resources and ability to collaborate with community service providers; excellent customer service and communication skills with the ability to discuss delicate matters with patients and providers; ability to work independently and as part of a multidisciplinary team;
strong organizational skills with a proven ability to prioritize and handle frequent changes in workload and manage competing priorities; strong problem‑solving skills; and demonstrated knowledge and skills necessary to provide care to patients through the life span with consideration of aging processes, human development stages, and cultural patterns in each step of the care process.
