A health services organization in Providence, RI, is seeking a Discharge Planner to assist in patient transition from hospital to post-discharge care. The candidate will facilitate referrals, communicate patient needs, and work with a multidisciplinary team. A Bachelor's degree in health services or related field is preferred, along with at least one year of relevant experience.

This role offers a pay range of $50,523 to $83,346. Full-time schedule includes Monday to Friday, with a day shift and possible weekend rotation.
