This position is eligible for a $2500 retention bonus.

The Providence Center Case Manager ACT Harmony Team will coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric symptoms.

• Coordinate a variety of services:
  Provide supportive counseling, problem solving, and limit setting.
• Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with people who have severe persistent mental illness and/or substance abuse issues, as it relates to case management.
• Monitor client status during spend down process, complete necessary DHS applications.
• In cooperation with Managers or Emergency Services Staff, assist with assessments, crisis interventions and risk management as needed.
• Negotiate and advocate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
• Transport consumers in own vehicle to planned activities and appointments.
• Complete recordkeeping in accordance with Health Information Services (HIS) and other TPC requirements within required time frames and abide by TPC confidentiality policy.
• Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
• Attend mandatory in-service trainings and other trainings required.
• Attend case presentations and conferences.

• Associate's degree required. Bachelor's degree from accredited four-year College or University in Human Services or related field preferred.
• Experience in Human Services field preferred.
• Must have valid driver's license and insured auto.
• Must be able to work flexible hours.
