Elmwood Adult Day Health Care Center is seeking a compassionate and dedicated Certified Nursing Assistant (CNS) to provide ongoing restorative and rehabilitative care to participants of our Centers. This person will be responsible for assisting in all aspects of patient care, such as ADL's/IADL's, and is expected to work closely with the care team and activities departments to promote positive, engaging and stimulating environments that work enhance the quality of life and support cognitive and physical function of the participants of Elmwood Adult Day Healthcare Centers in alignment with the mission, vision and philosophies of Elmwood Healthcare.

Key Responsibilities Direct Patient Care and Rehabilitative Activities- Assist participants with daily activities (ADLs) including eating, grooming, ambulating, toileting, and bathing.
- Support activities programs and help facilitate group and individual engagement.
- Provide restorative and rehabilitative care, including exercises and routines as needed.
- Assist with meals and snacks, monitor dietary intake, and help maintain CACFP food logs and meal counts.
- Ensure a clean, safe, and welcoming environment for participants.
- Observe, report, and document changes in condition or behavior.
- Foster dignity, respect, and confidentiality at all times.
- Work with interdisciplinary team members to address changes in participant conditions.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and ensure a cohesive approach to patient management.
- Communicate effectively with the interdisciplinary team to address any changes in patients' conditions or needs.
- Serve as a point of contact for staff and families regarding participant health and care.
- Foster open, respectful communication with participants, caregivers, and colleagues.
- Support staff in resolving participant care issues.
- Maintain accurate, detailed, and timely documentation of patient care, response to activities, and other tasks as mandated by those applicable policies of Elmwood Healthcare.
- Ensure documentation complies with company policies, insurance requirements, and regulatory standards.
- Adhere to infection control practices, safety protocols, and all relevant regulations to ensure a safe care environment.
- Use medical equipment and supplies safely and appropriately.
- Maintain strict confidentiality of all participant and organizational information.
- Adhere to HIPAA and all regulatory guidelines related to healthcare documentation and privacy.
- Complies and upholds the policies and procedures of Elmwood Healthcare and its local, state and federal regulatory authorities.
- Provide compassionate support to patients and their families, addressing concerns and offering comfort and reassurance as needed.
- Foster a positive and supportive relationship with patients to encourage adherence to care plans.
Participate in ongoing training and professional development activities.
QualificationsEducation: High-School Diploma and Certified Nursing Assistance Certificate required.
Licensure: Current, valid and unencumbered license to practice as a Certified Nursing Assistant (CNA) in the State of Rhode Island and Providence Plantations. No active, open or previously adjudicated disciplinary findings reported by the Rhode Island Department of Health, Board of Registration in Nursing or any other professional licensing Board therein.
Experience: Minimum of two (2) years of nursing experience in elder care, community health or long-term care settings.
Skills: Ability to cultivate and maintain an engaging and stimulating environment in group settings;
Excellent communication and interpersonal skills;
