Job Purpose : Provides personal care to residents according to written service plans.
E ssential
Duties and Responsibilities:
- Review the log book and assignment sheets for updates at the start of each shift.
- Assure that resident s are neat, clean , and properly clothed. Assist with bathroom and hygiene needs.
- Tidy apartment proactively assesses potential safety concerns.
- Proactive ly assess potential safety concerns in common areas of the building and report to appropriate parties when needed.
- Transports residents to and from activities and dining.
- Provides SAMM assistance within the allotted time frame (if applicable)
- Accurate documents are provided , including SAMM timely (if applicable)
- Give a verbal report of shift change to the following shift.
- Mealtime duties include mak ing morning coffee and serv ing breakfast as residents are ready. Serve lunch and dinner. Assist with setting tables. Encourage residents to take part in mealtimes . Assist residents with plate and utensil orientation. Clean up, load , and empty dishwasher, clean and disinfect tables and counters. Launder napkins and dish towels.
- Provide one - on - one support when needed to engage residents in activities or provide a quiet space to calm residents when required . Facilitates and assists with activity programming.
- Applies an e x cellent customer service approach in all aspects of the job.
- Committed to working with residents and families to develop and implement strategies that promote person-centered care.
Position Requirements:
- High School Diploma or equivalent required.
- Current Certification as a CNA or Home Health Aide in the state of w ork .
