Position Summary:
Directly work with clients and their families enrolled in the Family Service of Rhode Island’s Family Cedar Center. Complete Family Needs Assessments; develop action plans based on the Needs Assessments; identify resources; follow-up and coordination of care. Documentation and training as required by the certification and organizational standards.
Specific Duties / Responsibilities:
• Demonstrate ethical and collaborative behavior to ensure quality of care
• Demonstrate acceptance and support of family centered principles
• Establish rapport with families throughout the family’s participation in Cedar services
• Schedule evening and weekend meetings when needed by the family
• Provide a Diversity, Equity and Inclusion lens in service delivery
• Ability to transfer knowledge of the service system, availability of peer supports, accessing information concerning the child’s issues, and accessing public and private supports for the family
• Conduct needs assessment with family utilizing a standardized tool
• Assist family to set goals in their Family Care Plan
• Assist family in understanding their options within the care systems
• Coordinate care planning meetings
• Follow–up with families to determine progress, barriers in implementing their care plan and identify the need for revisions
• Provide health needs coordination for the purpose of ensuring effective coordination of the services being provided
• Document encounters with families, providers, community agencies and enters them into the permanent health record and other required data systems
• Demonstrate commitment to professional growth and maintenance of skills
• Attend supervision and other assigned meetings
• Completion of agency orientation and all mandatory training
• Other duties as assigned
Qualifications:
Bachelor’s degree preferred
Experience working children and families in the community from diverse populations
Experience working with children and families with special health care needs preferred
Bilingual preferred
Rhode Island Community Health Worker Certification preferred; certification required within the first 12 months of employment
Excellent communication skills
Ability to utilize an Electronic Medical Record and familiarity with Outlook, Word, Excel Ability to work in a hybrid environment while maintaining organizational PHI standards
Reliable transportation, current driver’s license, proof of automobile insurance and automobile registration
Bilingual/ASL skills are compensated by an additional 6%, above base pay.
Multilingual skills are compensated by an additional 8%, above base pay.
Physical Requirements:
• Travel to and from community and agency site locations, which could include using walkways, stairs, and /or elevators
• Ability to lift 20 pounds
• Ability to communicate effectively
