• Provides a well-focused psychosocial and discharge planning assessment of patient and family to develop a treatment plan and goals, appropriate to the age of the patient.
• In collaboration with the team, contributes to defining the need for further resources or services.
• Implements the treatment plan to achieve the treatment goals relative to age specific needs.
• Completes discharge planning and psychosocial assessment resulting in identification of the patient’s current and, where appropriate, past life crisis and probable discharge plan needs.
• Completes appropriate intervention plans on the interdisciplinary treatment plan within 72 hours.
• Coordinates with the treatment team in facilitating an optimal discharge plan for patients, including knowledge of general and age-specific resources including availability, eligibility, cost factors and types of service.

• Engages and communicates with patients and families in a direct, empathic manner, adapting approaches to age-specific sensory, cognitive, and social needs.
• Gathers, assesses, and interprets psychosocial data with awareness of age-specific factors such as signs of abuse, educational needs, and functional deficits.
• Advocates and communicates the family’s expectations and perspectives and the impact of stress and illness on their functioning.
• Identifies and contributes knowledge of the impact of ethnic, cultural, socioeconomic, gender, and sexual orientation factors on treatment and well-being.
• Demonstrates knowledge of different therapy modalities appropriate to the patient’s age and contributes to treatment and discharge planning in written and verbal form.
• Demonstrates ability to communicate data to the treatment team clearly and concisely; participates in team meetings and huddles as needed.
• Maintains current knowledge of community resources and provides accurate information to staff, patients, and families (including limitations, costs, and availability).

• Education - MSW
  : MSW or master’s degree in counseling.
• Training and Experience
  :
  Training and clinical experience in behavioral health; case management experience.

• Education - BSW
  :
  Bachelor’s degree in social work or other behavioral health specialty.
• Training and Experience
  :
  Training and clinical experience in behavioral health; case management experience.

• Mental Demands
  :
  Able to tolerate the stress of a fast-paced environment and work with multi-disciplinary staff and difficult patient populations.

• Reports to: Director of Social Services and Operations Manager
• Supervises: None, or Social Work Volunteers or social work students as assigned
• Other Relationships: Other Social Services staff, clinical unit-based staff, Unit Chief, and other team members
