Hospice Social Worker

Job Type: Per diem

Company: Athena Hospice of Rhode Island

We service the entire state of Rhode Island. Up to $45 an hour, depending on license and experience, with weekly pay. Looking for someone to help up to 20 hours a month.

As a per diem licensed social worker, you will play a vital role in planning, organizing, and directing all required social services to meet the needs of our residents. Your expertise will ensure that the medically related emotional and social needs of each individual are met and maintained on an individual basis.

Responsibilities:
  • Assess the psychosocial status of patients and families/caregivers
  • Provide an assessment of the patient's identified residence
  • Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, and plans intervention based on evaluation findings
  • Counsel patients and family/caregivers as needed in relation to stress and other identified coping difficulties
  • Educate the team on any special needs related to the culture of the patient and family
  • Participate in the development of the individualized plan of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings
  • Assist the physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues
Requirements:
  • Bachelor's in social work or related field required
  • State social work license required
  • One (1) year of clinical healthcare and/or hospice experience preferred
  • An understanding of hospice philosophy and issues of death/dying preferred
  • Must be a licensed driver with an insured automobile
  • Must be able to read, write, speak, and understand the English language
  • Must possess the ability to make independent decisions when circumstances warrant such action
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and general public
