. We are seeking a dedicated Registered Dietician (RD) to help us deliver outstanding care to our residents.
- Develop personalized meal plans
- Provide nutrition counseling and education to patients on healthy eating habits and lifestyle changes
- Monitor and evaluate patients' progress and adjust meal plans as needed
- Collaborate with healthcare professionals to develop comprehensive treatment plans for patients with specific medical conditions
- Stay up-to-date with the latest research and developments in the field of nutrition and dietetics
- Provide health coaching to individuals or groups on topics such as weight management, diabetes management, and heart health
- Conduct diagnostic evaluations to identify nutritional deficiencies or imbalances
- Develop and implement tube feeding plans for patients who are unable to consume food orally
- Licensed/ Certified Registered Dietitian in Rhode Island
- Analyzing and documentation of admissions, high-risk residents, tube-feeders, dialysis and weight loss. Documentation to include goals, dietary restrictions, tracking clients' dietary intake and progress.
- Providing education to residents, on how to maintain a balanced diet and optimum health.
- Knowledge of RAI process, MDS, CAA and Care Plans
- Knowledge of HIPAA, to ensure privacy and to safeguard resident information.
