Overview
The Nurse Practitioner is accountable to facilitate a quality hospital experience and outcome for patients within a cost-effective time frame. The Nurse Practitioner will be responsible to the Medical Director of Patient Services and responsible for overseeing the progress of a specific caseload of patients across the care continuum to include outpatient clinics, specialty clinics, pre-admission and post-discharge when feasible. Acts as an educator, consultant, and liaison to healthcare providers caring for the patients across the care continuum.
Responsibilities- Evaluating patients in Genetic practice in clinic and in-patient hospital setting.
- Coordinating patients' care, treatments and testing.
- Ordering appropriate tests and/or medications as needed.
- Managing emergencies on site and via phone, telephone triage and supportive care as needed.
- Work closely with the team of 6 genetic counselors and 2 metabolic dieticians.
Minimum qualifications include licensure as a Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing. Master of Science Degree, graduate of an accredited Nurse Practitioner program; specialty certification required;
Baccalaureate in Nursing. Certified in Basic Cardiac Life Support (BLS).
Experience:
Two years related experience as an RN in an acute care hospital setting and preferred 1-2 years experience as a nurse practitioner.
Additional knowledge/experience:
- Experience managing individuals with genetic conditions including high-risk surveillance, or complex patients preferred.
- Genetics-focused training not required but willingness to learn would help with a steep learning curve.
General hospital environment with occasional stressful conditions associated with patient care. Independently responds to patient-care situations with knowledge and skills acquired through education, certification and work experience. Provides leadership in development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model. Oversees care coordination in acute, home care, and ambulatory care settings as feasible.
Assists the healthcare team to achieve healthcare goals by providing necessary support, education and encouragement. Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics. Provides direct care to patients in acute care, ambulatory care and home care settings. Performs advanced practice skills under the supervision of the Medical Director. Provides expert consultation for clinical issues to promote increased knowledge and skills for the consultee.
Participates in education, clinical research and mentoring of healthcare providers. Facilitates patient/family education in area of expertise. Annually completes mandatory safety infection control, BLS, and ACLS/PALS. Prescriptive privileges per licensure.
