As a Senior Community Care Clinician (Nurse Practitioner or Physician Assistant), you provide high‑risk patient care in home, nursing home, assisted living and senior housing settings to improve access to care across settings.
Primary Responsibilities- Conduct comprehensive assessments, including past medical history review, medication reconciliation, vital signs, and physical exam.
- Perform evidence‑based screenings and point‑of‑care testing (as appropriate).
- Support members during short‑term sub‑acute rehabilitation episodes.
- Ensure accurate and complete documentation, including ICD-10 conditions.
- Communicate and collaborate with the interdisciplinary care team.
- Conduct advanced illness and advanced care planning conversations.
- Provide patients and caregivers with counseling and education.

Required Qualifications- For Nurse Practitioners:
Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care. - For Physician Assistants:
Current NCCPA certification and state licensure. - Active, unrestricted licensure in the state of practice (or ability to obtain by start date) without disciplinary actions or practice limitations.
- Current, active DEA licensure/prescriptive authority or ability to obtain post‑hire as required by state regulations.
- Ability to obtain a collaborative practice agreement, if applicable in your state.
- Ability to navigate varied environments; sit, stand, or kneel as needed to perform duties.
- Active and unrestricted driver’s license with reliable transportation access.
- 1+ years of clinical experience as an Advanced Practice Clinician (long‑term care setting preferred).
- Experience in geriatric medicine, long‑term care, senior living, or home care settings.
- Experience meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
- Understanding of geriatrics and chronic illness.
- Understanding of advanced illness and end‑of‑life discussions.
- Proficiency with electronic medical records and technology.
Compensation for this specialty generally ranges from $109,500–$164,000, including base pay and bonus. Total cash compensation is determined by local labor markets, education, experience, and performance. In addition to salary, benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution, all subject to eligibility requirements.
