Job SummaryWe are seeking an experienced and clinically strong Registered Nurse (RN) to serve as our Infection Preventionist & Wound Care Nurse. This full‑time position offers an exciting opportunity for a dedicated nursing professional to oversee the facility's Infection Prevention and Control Program while coordinating and managing wound care services throughout the center. As a key member of our team, you will collaborate closely with nursing leadership, physicians, and the interdisciplinary team to promote resident safety, prevent infections, improve wound healing outcomes, and ensure compliance with all federal, state, and facility regulations.
ResponsibilitiesInfection Prevention & Control
Lead and maintain the facility's Infection Prevention and Control Program
Conduct infection surveillance, monitoring, tracking, and trend analysis
Investigate infections, outbreaks, and exposure concerns
Implement infection prevention strategies and corrective action plans
Ensure compliance with CDC, CMS, OSHA, RIDOH, and facility infection control standards
Educate and train staff on infection prevention practices, hand hygiene, PPE use, and isolation precautions
Support and monitor antibiotic stewardship initiatives
Maintain required infection prevention documentation, reports, and logs
Participate in QAPI, infection control committees, and performance improvement activities
Perform comprehensive skin assessments and ongoing wound evaluations
Develop, coordinate, and oversee wound treatment plans
Monitor wound healing progress and treatment effectiveness
Ensure timely and accurate wound documentation in accordance with regulatory requirements
Collaborate with physicians, consultants, and the interdisciplinary team regarding wound care interventions
Monitor residents for changes in skin integrity and implement preventative measures
Provide education and support to nursing staff regarding wound prevention and treatment protocols
Ensure compliance with clinical standards, infection prevention practices, and quality measures related to wound care
Current Rhode Island Registered Nurse (RN) license required (Compact License accepted)
Long‑term care or skilled nursing experience preferred
Infection Prevention experience preferred
Wound Care experience preferred
Infection Prevention certification and/or Wound Care certification preferred
Strong understanding of infection control regulations and best practices
Excellent assessment, documentation, organizational, and communication skills
Ability to analyze clinical data and implement quality improvement initiatives
Ability to work independently and collaboratively within an interdisciplinary team
