SUMMARY

The Stroke Program Nurse Navigator is a registered nurse who reports to the Director of Emergency Services and is responsible for supporting clinical teams throughout the hospital who provide direct care to stroke patients, and their families. Patient navigation refers to the assistance offered to patients and their families to help them move through the complexities of the healthcare system and overcome any barriers to quality care.

The Stroke Program Navigator recognizes that every patient and every stroke is unique, and that care must be individualized. The candidate will work closely with the Stroke Program Manager and function as a point person for patients and families during and following their hospitalization.

RESPONSIBILITIES
  • Assume a key role in assuring care on a continuum, coordinating diverse resources and patient care activities for individuals receiving patient‑centered stroke care; act as the point person for patients and families/caregivers throughout their hospital stay and following discharge.
  • Work collaboratively with the interprofessional team to ensure an evidence‑based plan of stroke care.
  • Provide in‑person support for the clinical care of patients; serve as an expert stroke resource for nurses and providers; assist with patient assessment; coordinate patient care using defined protocols; provide emotional support to patients and families; provide patient and family education.
  • Complete NIHSS; intervene in real‑time quality measures; conduct chart reviews; document within patients’ electronic health record; collect and manage data for regulatory compliance and accreditation; provide EMS feedback; provide patient discharge follow‑up; collaborate with healthcare team to create discharge plan; provide staff education; participate in quality initiatives and research; serve on committee membership.
MINIMUM QUALIFICATIONS
  • Graduate of an accredited school of nursing; associate’s degree in nursing required; bachelor’s degree in nursing preferred.
  • Current license to practice as a Registered Nurse in the State of Rhode Island.
  • Basic life support (BLS) certification through AHA; ACLS within six months of hire.
  • NIH Stroke Scale (NIHSS) certification within six months of hire.
  • Stroke certification within two years of hire.
  • Understanding of the provisions of the Nurse Practice Act of the State of Rhode Island.
  • Demonstrated knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages, and cultural patterns in each step of the care process.
EXPERIENCE
  • Experienced nurse with 3‑5 years of acute care experience; stroke patient care experience strongly preferred.
  • Strong interpersonal skills to effectively function as a member of the healthcare team to coordinate and discuss patient care activities.

$77,230.40 - $
