Job Overview
Under the supervision of the Brown University Health Cardiovascular Institute Clinical Manager, the Advanced Heart Failure Nurse coordinates care from initial referral through the care continuum for patients with advanced heart failure, referral for advanced therapies, VAD care, post‑transplant care, and amyloid and HOCM specialty clinics. The RN, as a member of a multi‑disciplinary team, facilitates the provision of quality, evidence‑based, ethical, safe, compassionate, and cost‑effective care and performs all essential functions with consideration of the age‑specific needs of the geriatric and adult patient.
The RN fosters patient‑ and family‑centered care in accordance with the Brown University Health mission, vision, and values, with consideration of the life span, including all developmental stages and cultural patterns.
- Monitors the patient schedule and ensures that physician and patient needs are met.
- Participates in a team‑based approach to patient care.
- Develops relationships with patients, helps identify patient needs, provides ongoing education, coordinates health‑care services, assists in finding needed resources and facilitates referral, supports patient self‑management of disease, and participates in ongoing follow‑up and coordination of care.
- Facilitates and maintains ongoing communication with shared‑care team members at Tufts Medical Center to facilitate appropriate advanced therapies referrals and assure appropriate post‑VAD and post‑transplant care transition.
- Assists with coordinating maintenance and repair of specialty‑specific equipment and supplies.
- Maintains quality assurance, safety, environmental, and infection‑control standards, in accordance with established hospital department policies, procedures, and objectives.
- Performs other duties as assigned.
- Licensure as a Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing, or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
- Completion of formal education in an accredited school of nursing;
Bachelor’s Degree in Nursing preferred. - Registered Nurse with current license to practice in the State of Rhode Island.
- BLS Certified.
- Advanced Cardiac Life Support (ACLS) certification required within six (6) months of employment.
- Demonstrated knowledge and skills necessary to provide care to patients throughout the lifespan, with consideration of aging processes, human development stages, and cultural patterns in each step of the care process.
Two to three years of relevant cardiology, critical care and/or emergency room experience with demonstrated clinical skill.
Work Environment andPhysical Requirements
Ambulatory clinic environment; works with medical equipment and has direct patient contact. Any potential hazards are minimized by adherence to established safety procedures and precautions, including the use of safety equipment and protective clothing.

Pay Range$35.70–$71.40 per hour.
