In-Home Health - Nurse Practitioner or Physician Assistant; Per Diem-Providence, RI

Overview

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. The role involves visiting patients in their homes, conducting medication and medical history reviews, a physical evaluation, and coordinating diagnostic tests as ordered by the health plan.

• Conduct 45‑minute visits in patients’ homes, connecting one‑on‑one and answering health questions.
• Review medication and medical history, perform a physical examination.
• Order or coordinate diagnostic tests such as diabetic eye exams, spirometry, bone density screening, etc., as requested by the patient’s health plan.
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Provide all Signify Health services, including Diagnostic & Preventive Services products.
• Conduct virtual visits and travel locally and within licensed states as needed.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Eligible to cover primary locations:
  Providence, RI.
• Per diem, part‑time hours around 12 per week.

• Hourly base rate plus incentive structure per visit completed.

The typical pay range for this role is $0.00 – $99.14 per hour. The actual base salary will depend on experience, education, geography and other relevant factors.
