FSRI seeks a candidate who wants to make a positive impact on the community by providing care coordination services to young adults living in the program. The Care Coordinator advocates for residents and families, navigates human services and agencies, and collaborates with providers such as DCYF, schools, medical providers, and other service providers to ensure appropriate delivery of services to residents.
PositionSummary
Responsible for providing care coordination services to young adults living in the program, advocating for residents and their families, and collaborating on cases with a multitude of providers to ensure appropriate delivery of services.
Qualifications- Bachelor’s degree in a human services field preferred, or have the qualifications to become a Certified Community Health Worker.
- Experience in child welfare, youth programming, youth development, or related fields.
- Excellent verbal and written communication skills.
- Knowledge of and sensitivity to cultural, ethnic, racial and socioeconomic issues.
- Valid driver’s license, reliable transportation and proof of automobile insurance and registration.
- Ability to work shifts less than three hours in any workday and attend required staff meetings as agreed in advance.
- Bilingual or ASL skills compensated by an additional 6%, above base pay.
- Multilingual skills compensated by an additional 8%, above base pay.
- Travel to and from community locations and residential site locations, including use of walkways, stairs and/or elevators.
- Ability to lift up to 20 lbs.
- Ability to push or pull 10 lbs or more.
- Ability to communicate effectively.
